The form is 100% confidential and secure.  Although you are asked to give your name it is simply to ensure that we have all trainees completing the form. Your scores and comments will be collated with those of other respondents undertaking the same attachment.  Questionnaires from more than one 4 month period will be combined before making feedback available to those organising the attachments.  It will not therefore be possible to identify an individual respondent.

The feedback forms require you to complete all the relevant questions in the first section. These are: Your Name, Specialty, Consultant supervisor's surname (Foundation only), Your GMC number, Training location, Date at start of placement and Placement length in weeks. If any of these sections are not completed the system will record validation errors and will refuse to submit. If this happens please ensure that each section is properly completed, before resubmitting.

Please also note that the comment boxes on the final page only record a maximum of 40 words despite being much larger than this!
